Information sheet for women
You will be asked to sign a consent form - the Cervical Cytology Form - when you go for your free CervicalCheck smear test. This consent allows CervicalCheck to receive, hold and use your personal details and information about your smear test sample. This may include past smear test samples and colposcopy results.
The Cervical Cytology Form has an information sheet attached to it explaining the smear test process and why your consent is needed. It is available in 11 languages and can be downloaded by clicking on the links below.

Irish Cancer Society Support Publications
